TrueMed Home Health is seeking compassionate Licensed Vocational Nurses (LVNs) for intermittent home health visits throughout several counties. Responsibilities:
- LVN provides patient care according to the Plan of Care and under direct supervision of RN Case Manager.
- Support quality improvement practices.
- Monitor reactions and patient progress using observation, assessment, and evaluation skills.
- Educate patients and family members according to disease process, medications, POC, treatment options, and home care procedures according to the plan of care.
- Demonstrate organizational and time management skills.

- Full Time caseload is 5 to 6 visits per day when available.
Qualifications/Requirements:
- A current unrestricted California LVN license
- Must possess reliable transportation, valid CA driver's license, and a proof of auto insurance
- Current CPR and health clearance
- Must have access to a computer to complete patient notes
- Must be detail-oriented and possess strong communication skills
- Bilingual Preference (Russian, Mandarin, Cantonese, Tagalog)
- IV & Wound Care Certifications a plus
Pay Rate: $40 - $50/per visit
