Primary Responsibilities:
1. Community Engagement:
- Identify and engage potential participants for the ECM program through outreach efforts such as community events, referrals, and direct interactions.
- Represent Abitare Inc. at community events, health fairs, clinics, and meetings.
2. Program Promotion:
- Educate individuals and groups about healthcare programs and services offered by the organization and enroll patients.
- Develop and distribute promotional materials such as brochures, flyers, letters, and social media posts.
3. Client Support:
- Assist clients in accessing healthcare services, including providing referrals and follow-ups.
- Act as a liaison between healthcare providers and the community to address barriers to care.
- Focus on building relationships with nursing home administrators, staff, and residents.
4. Partnership Development:
- Establish and maintain partnerships with local organizations, clinics, and community groups.
- Collaborate on joint initiatives to expand outreach efforts.
5. Data Collection and Reporting:
- Track outreach activities and maintain records of community interactions and outcomes.
- Provide regular reports to the program manager, including success metrics and areas for improvement.
6. Enrollment Support:
- Screen individuals for eligibility and guide them through the ECM enrollment process.
- Assist participants in completing required forms and obtaining necessary documentation.

- Conduct in person visits within nursing homes presenting Enhanced Care Management program to residents, and Initial Risk Assessments.
- Develop targeted promotional materials and educational sessions aimed at nursing home communities.
- Coordinate with internal and external teams to ensure timely and accurate enrollment.
Qualifications:
- Experience in community outreach, healthcare, or social services (typically 1-3 years).
- Strong communication and interpersonal skills.
- Knowledge of healthcare systems and local community resources.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite; familiarity with CRM tools is a plus.
- Bilingual Skills preferred Spanish, Tagalog.
