Job Overview – Enhanced Care Management (ECM)
Position Summary: The Lead Care Manager is responsible for providing comprehensive care coordination and case management services to eligible members within the Enhanced Care Management (ECM) program. This role ensures seamless service delivery, non-duplication of services, and supports members in achieving optimal health outcomes through personalized care plans and community resource connections.
Key Responsibilities:
- Member Engagement: Identify and engage eligible members, offering ECM services in accessible locations, including members' residences or preferred settings, in accordance with Managed Care Plan (MCP) guidelines.
- Care Coordination: Oversee the provision of ECM services, ensuring the effective implementation of individualized care plans tailored to members' physical and psychosocial health needs.
- Service Integration: Collaborate with healthcare professionals, social services, and community resources to connect members with necessary support, such as transportation and other social determinants of health.
- Advocacy: Act as an advocate for members, facilitating communication with healthcare providers and ensuring that members' preferences and needs are addressed.
- Health Promotion: Utilize motivational interviewing, trauma-informed care, and harm-reduction approaches to support members in health behavior changes and self-management.
- Discharge Planning: Coordinate with hospital staff to develop and implement effective discharge plans, ensuring continuity of care post-discharge.
- Treatment Adherence Monitoring: Monitor members' adherence to treatment plans, including medication management, and provide education to promote compliance.
- Documentation: Maintain accurate and timely documentation of all member interactions, care plans, and services provided, ensuring compliance with organizational and regulatory standards.
Qualifications:
- Education: Bachelor's degree in Social Work, Nursing, Psychology, or a related field; Master's degree preferred.
- Experience: Minimum of 2-3 years in community health or social services, with at least 2 years in case management or care coordination; experience with ECM or similar programs is highly desirable.
- Skills:
- Strong understanding of care management principles and the healthcare delivery system.
- Proficiency in motivational interviewing, trauma-informed care, and harm-reduction techniques.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Strong organizational and time-management abilities.
- Proficiency in Microsoft Office Suite and electronic health record systems.
- Certifications: Valid driver's license and willingness to travel to community locations as required.
Preferred Qualifications:
- Bilingual proficiency in English and another language commonly spoken by the member population (e.g., Spanish, Tagalog).
- Familiarity with Medi-Cal and CalAIM initiatives.
Work Environment: This position involves both office-based work and fieldwork, including home visits and attendance at medical appointments with members. The Lead Care Manager must be adaptable to various community settings and capable of managing a flexible schedule to meet members' needs.
Physical Requirements: The role requires the ability to travel to various locations, conduct home visits, and accompany members to appointments. The Lead Care Manager should be able to perform these duties with or without reasonable accommodations.
