Initial Requirements:
- Must have Masters Degree in Social Work
- Must have Home Health experience
- Minimum one year clinical experience (two+ years preferred)
- Current license to practice in CA
- Current CPR certification
- Current CA driver's license plus proof of car insurance.
- Computer/Internet experience (web-based charting to be used)
- Bi/Multi-lingual skills a plus (e.g. Spanish, Farsi, Tagalog, Chinese etc.)
Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Provide clinical counseling and behavioral therapy to patients, utilizing techniques such as cognitive behavioral therapy and motivational interviewing.
- Ensure compliance with HIPAA regulations while maintaining accurate medical documentation and records.
- Collaborate with healthcare teams to coordinate patient care and facilitate home health services.
- Engage in crisis intervention and support families through child and family counseling as needed.
- Maintain proficiency in medical terminology and computer skills for effective patient management.
