What You’ll Do

• Perform in-home physical therapy evaluations and treatments

• Provide high-quality, one-on-one patient care

• Complete documentation efficiently using TherapyBoss EMR

• Communicate with referring home health agencies as needed

• Practice without pressure to “hit productivity numbers”

What We’re Looking For

• Active Illinois Physical Therapist license

• 1+ year experience in home health, outpatient, SNF, or hospital settings preferred

• Comfortable working independently and managing your schedule

• Strong communication and organization skills

• Comfortable with electronic documentation

Bonus Qualifications

• Bilingual (Spanish, Tagalog, Vietnamese, etc.)

• Experience with orthopedic, neurologic, or post-operative patients
