Initial Requirements:

Current license to practice Occupational Therapy in CA
Current CPR certification
Current CA driver's license plus proof of car insurance.
A minimum of 1 year clinical experience (two+ years preferred, previous home health experience is a PLUS)
Computer/Internet experience (online charting to be used)
Bi/Multi-lingual skills are highly desired (e.g. Spanish, Russian, Tagalog, etc.)

Responsibilities

Conduct comprehensive assessments of patients in home health settings to develop personalized care plans.
Provide therapeutic interventions to enhance patients' abilities in daily living activities (ADLs).
Collaborate with multidisciplinary teams to ensure effective discharge planning and continuity of care.
Educate patients and families on treatment plans, exercises, and health management strategies.
Maintain accurate documentation of patient progress and treatment outcomes while adhering to medical terminology standards.
Utilize computer skills for scheduling, reporting, and communication purposes.

Job Types: Part-time, Per diem

Pay: $140,000.00 - $164,000.00 per year
